Follow I-235 S to NE 10th St (Exit 1G)
Turn left onto NE 10th St
Turn left onto N Lincoln Blvd

1. TURN RIGHT ONTO 13th St
   Turn right JB Saunders Ave.
2. PARK IN GARAGE P1
   Reserved parking for OUMC Clinics can be found on Level 1. If you park on a different level, take the garage elevator to Level 1. Enter the OUMC Clinics building on Floor 1.
3. TAKE ELEVATOR E TO FLOOR 5
4. FOLLOW SIGNS TO SUITE 524 PRE-ANESTHESIA UNIT

Items to Bring With You:

• Your driver's license or other official ID.
• Your health plan card(s).
• Please bring a list of all your medications and dosages.
• Please plan for 1 to 2 hours for this appointment. You will
  be completing your pre-operative testing and anesthesia
  assessment to help prepare for your day of surgery.
• If you are unable to keep this appointment, or need to
  reschedule, please call the number above.

1. DOBLE A LA DERECHA EN LA CALLE 13th
   Doble a la derecha en la calle JB Saunders
2. APARECER EN GARAJE P1
   El estacionamiento reservado para OUMC Clinics
   se puede encontrar en el Nivel 1. Si se estaciona
   en un nivel diferente, tome el elevador del garaje
   hasta el Nivel 1. Ingrese al edificio de OUMC
   Clinics en el Piso 1.

• Su licencia de conducer u otra identificacion oficial.
• Su tarjeta del plan de salud.
• Por favor traiga una lista de todos sus medicamentos y
  dosis.
• Por favor planifique de 1 a 2 horas para esta cita. Usted
  completará sus pruebas preoperatorias y la evaluación
  de anestesia para ayudar a prepararse para su día de
  cirugía.
