Eligibility Requirements

When you enroll for Live On Home Cooking!, be prepared to commit to all four sessions in the series. The program is part of a health assessment conducted by the OU College of Medicine Department of Endocrinology. Upon enrollment, we will schedule you for two individual consultations with an HHDC Registered Dietitian -- one before the first session of the series, and one after the final session. We do require a referral from the primary-care physician who treats your diabetes, and we can assist in acquiring your doctor's referral. The brief consultations include a brief survey of height, weight, blood pressure, and A1C, along with an assessment of your food choices and cooking knowledge/habits. Finally, because the four group sessions will be professionally filmed, your participation requires signing a release allowing your image to appear.

This program is offered at no cost to participants, thanks to the generous support of HHDC donors and partners.

Call (405) 271-1000, press "1" for appointments, and ask to enroll in the Live On Home Cooking! workshops beginning April 18th.
