To enroll in PreventT2, you must a) have a BMI of 25 or higher and b) have prediabetes. Prediabetes is defined as an A1C level between 5.7 and 6.4 percent or a fasting blood sugar level from 100 to 125 mg/dL (110 to 125 mg/dL for Medicare recipients). | Calculate your BMI now
A documented A1C or fasting glucose from your physician’s office is required for Medicare recipients. All other insurance plans do not require documentation.
If you are a female with a history of gestational diabetes and a current BMI of 25 or higher, you are also eligible. No documentation of gestational diabetes is required. | Calculate your BMI now
The value of the PreventT2 program is estimated at $500. HHDC is able to offer the program at a discount, because of substantial support from our donors.
MEDICARE RECIPIENTS: Medicare covers 100% of PreventT2 enrollment with no out-of-pocket cost to you. This benefit is available once in your lifetime.
PRIVATELY INSURED: Many major private insurers cover PreventT2 as a diabetes-prevention benefit. Contact your insurer to determine what your plan will cover. If your plan does not cover PreventT2 in any amount, your cost to enroll is $249, payable in full by personal check or debit/credit card.
MEDICAID/SOONERCARE or NO INSURANCE: At this time, Medicaid/SoonerCare does not cover any portion of diabetes prevention programs like PreventT2. Your cost to enroll is $249, payable in full by personal check or debit/credit card.
OUHSC EMPLOYEES: Healthy Sooners offers a $100 reimbursement incentive for any employee who meets his/her weight-loss goal and completes the one-year program. | Reimbursement Form (required) | Reportable Income Form (required)

TERMS OF PARTICIPATION
For maintaining your progress in PreventT2, it is essential that you attend as many of the sessions as you can. Once you are enrolled and your payment is made, there are no refunds.
